Application Instructions- 

Only fill out this application after you have applied for Universal Preschool through upk.colorado.gov .

Children must be 3 or 4 years old by October 1 to be eligible for the current school year. If your child turns 3 after October 1, please apply in the spring for the following year. If your child's 5th birthday is on or before October 1 this year, please enroll them in kindergarten at D11.org/Enrollment.
