If a student or visitor is involved in an incident on district property or during a school-sponsored activity, please follow the steps below to report it:
Reporting Instructions:

Include all relevant details, including:

Date and time of the incident

Location
-
Description of what occurred

Names of individuals involved or witnesses

Submit the completed form to your school administrator or directly to the Risk Management Department at RiskMgnt@D11.org
