Student/Visitor Incident

 

If a student or visitor is involved in an incident on district property or during a school-sponsored activity, please follow the steps below to report it:

Reporting Instructions:

  1. Complete the Student/Visitor Incident Report Form
     

  2. Include all relevant details, including:

    • Date and time of the incident

    • Location

    • Description of what occurred

    • Names of individuals involved or witnesses

  3. Submit the completed form to your school administrator or directly to the Risk Management Department at RiskMgnt@D11.org

Instructions:

  1. Open the form link

  2. Go to File > Download

  3. Print the form

  4. Complete all required fields

  5. Return the completed form to Risk Management following the instructions provided
