Employee Leave

Employee Leave Request   As part of its benefits package, School District 11 offers employees options to request leave of absence for limited reasons.  An employee requesting a leave of absence for more than 10 consecutive workdays must first print the Employee Leave of Absence Request form and complete the form to submit to his/her supervisor. Once acknowledged by the supervisor, the employee then submits the form to the Human Resources Department.  Please read all instructions and disclosures when completing this form.
