CSSD11 Food & Nutrition Services (FNS) procedure for Accommodating Children with Special Dietary Needs

Facts from the Food Allergy and Anaphylaxis Network:

  • Eight foods account for 90 percent of all reactions in the U.S.: milk, eggs, fish, crustacean shellfish, wheat, soy, peanut, and tree nuts (walnuts, almonds, cashews, pistachios, pecans, etc.)
  • Food allergies result in 30,000 emergency room visits annually and 150 to 300 deaths
    There is no cure for a food allergy; only strict food avoidance can prevent a reaction

CSSD11 FNS will make meal modifications prescribed by a licensed physician, advanced practice nurse with prescriptive authority, or physician’s assistant to accommodate a disability. Properly completed and signed paperwork is required.

A disability is considered a physical or mental impairment which substantially limits one or more major life activity.
It is strongly recommended that a recognized medical authority annually update the prescribed diet order.

CSSD11 FNS will not make school meal modifications for requests that do not rise to the level of a disability including;

  • Food sensitivity that can be accommodated through menu choices
    Note: Milk is not a required meal component and can be declined by any student
  • Dietary preferences for religious, ethical, or cultural reasons, or general health concerns

To discontinue a diet order on file in our office, CSSD11 Food & Nutrition Services must receive this form:
