• New Student Enrollment Form - Only students new to District 11
    Complete the online pre-enrollment form. When you get to the last page, Print the form and be sure to click Submit. Bring the form to your school(s) to enroll your student(s). Refer to the student enrollment page for additional documentation required to enroll your child(ren).
    Returning Student Form
    If your child previously attended a District 11 school, and is re-enrolling after attending a different district.
    Returning Student Form: English | Spanish 
    Transferring from one District 11 school to another District 11 School
    Complete each of the following forms:
         Withdrawal Form: English  |  Spanish  (return to school currently attending) 
         In-District Transfer/Enrollment Update Form: English |  Spanish (return to new school)
