NOMINATION GUIDELINES

In the space provided below (or on the online request form), please write in ink pen or word process a narrative and respond to as many of the items as you are able. Use the front and back of the form or the equivalent of one typed page. Specific and expansive nominations are most likely to be successful, so be as complete as you can. Then add any additional or personal remarks, as you like. Nominations become the property of the Rotary Club of Colorado Springs and may not be copied.

Describe or give examples of the characteristics of the teacher you are nominating considering:

• Ability to communicate with students and presentation of his/her subject matter

• Influence on you and your fellow students

• Extra efforts to further the education of students, adaptation to individual differences

• Preparation of students to be productive contributing members of society

• Communication of values, expectations and fairness

• Participation in extra activities such as sports and clubs.

• Describe favorite class experience
