NOMINATION GUIDELINES

In the space provided below, please write in ink pen or word process a narrative and respond to as many of the items as you are able. Use the front and back of this form or the equivalent of one typed page. Specific and expansive nominations are most likely to be successful, so be as complete as you can. Then add any additional or personal remarks, as you like. Nominations become the property of the Rotary Club of Colorado Springs and may not be copied.

Describe or give examples of the characteristics of the teacher you are nominating/considering:
