Kindergarten registration at Midland:
Original Birth Certificate
Immunization Record
Social Security Number
Proof of
Residence (i.e., phone bill, utility bill, etc. that has student's address)
New Colorado Department of Public Health
immunization requirement:
Children need to have a 2nd
varicella (chicken pox) vaccination prior to the start of kindergarten
1 Book Bag or Back Pack (with name)
1 Pair of scissors (Fiskar)
1 Box of regular crayons (8 or 16 colors
only)
1 box Regular #2 pencils
* SHARPENED
4 Large Glue Sticks
1 Box of fat markers (8-standard colors)
1 Box of sandwich size bags - with zippers
1 Box of gallon size bags - with zippers
1 Pack white index cards (at least one side blank)
1 Large box of tissues
$4.00 for school agenda
(make checks payable to Midland International)
Ms. Pelletier's class also requires a bath sized
towel with their name on it.
