Kindergarten Registration

Children must be five years old by October 1, 2009

The following records are required for Kindergarten registration at Midland:

Original Birth Certificate
Immunization Record
Proof of Residence (i.e., phone bill, utility bill, etc. that has student's address)

New Colorado Department of Public Health immunization requirement:
Children need to have a 2nd varicella (chicken pox) vaccination prior to the start of kindergarten

 

KINDERGARTEN SUPPLY LIST for 2009 -2010

 

1 Book bag or backpack (with name)

1 Pair of scissors (Fiskar preferred because they last longer)

1 Box of regular crayons (8 or 16 colors only)

1 Box regular #2 pencils * SHARPENED

4 Lg. glue sticks

1 Box of fat washable markers (8-standard colors)

1 Box of sandwich size bags - with zippers

1 Box of gallon size bags - with zippers

1 Pack white 3x5 index cards (at least one side blank)

1 Lg. box of tissues

1 8oz. or 16oz. water bottle (with name)

Ms. Pelletier's class also requires a bath sized towel with student’s name on it

 $4.00 for school agenda (make check payable to Midland International Elementary)
