New Student
Registration for the regular school year is handled in our
counseling office from 8:00 a.m. to 4:00 p.m. Please allow
approximately 30 minutes to register your child. For
information, please call 328-2306.
Mann Middle School
requires the following information to register:
1. Proof of residency
(contract, lease, rental agreement, utility bill)
2. Home phone
3. Work phone
4. Local emergency phone
(in case of accident or illness)
5. Proof of Immunizations
6. Withdrawal form from previous school
7. Complete address of previous school
8. Social Security Card of student
9. Birth Certificate
10. Current I.E.P. (for Special Education)

For Kindergarten through 8th Grade you will need the
following:
* Copy of immunization record (including Hepatitis B)
* Copy of Social Security Card
