Birth Certificate
Immunization Records
Child's Social Security Number
Proof of Residence
Registration
packets will be available in the school office on April 6th. Packets
will not be accepted before April 24th.
New Colorado Department of
Public Health immunization requirement:
Children need to have a 2nd varicella (chicken pox) vaccination prior to the
start of Kindergarten.
