Program Information
1. Colorado
State Law requires that incoming Kindergarten students must have the following
immunizations/shots prior to entering school:

5 DTP/DTaP
(Diphtheria/Tetanus/Pertussis)*
-
4
Polio*
-
3
Hepatitis B (a three-shot series over six months)
-
2 MMR
(Measles/Mumps/Rubella)

Varicella (Chicken Pox) OR a History of Chicken Pox confirmed in
writing by your health care provider.
* The number
of DTP and polio immunization may vary depending on the age of the child
when the shots were given.

requirement).
2. Colorado State Law requires
that students entering 6th and 10th Grades must have the
TdaP (Tetanus/Diphtheria/Acellular Pertussis) immunization/shot
prior
to the start of school.
3. State
law requires all students to have either an up-to-date immunization
document or a signed exemption on file. Parents may sign a
personal or religious exemption available in the school office, or they
can contact their physician to obtain a medical exemption. In the
event of an outbreak, exempted persons may be subject to exclusion from
school and to quarantine. |
