DISTRICT 11 IMMUNIZATION INFORMATION

No student shall be permitted to attend or continue to attend any school in this District without first meeting legal immunization requirements or obtaining exemption from such requirements for health, religious, or personal reasons, as provided by law.

Colorado State Immunization requirements have been changed for some students beginning in Fall 2007-2008 School Year.  

1. Colorado State Law requires that incoming Kindergarten students of the 2007-2008 school year must have the following immunizations/shots prior to entering school: 

  • 5 DTP/DTaP (Diphtheria/Tetanus/Pertussis)*

  • 4  Polio*

  • 3  Hepatitis B (a three-shot series over six months)

  • 2  MMR (Measles/Mumps/Rubella)

  • ** 2 Varicella (Chicken Pox) OR a History of Chicken Pox confirmed in writing by your health care provider. 

*  The number of DTP and polio immunization may vary depending on the age of the child when the shots were given.

2.  2007 Colorado State Law requires that students entering 6th and 10th Grades, in August 2007, must have the following NEW immunization/shot prior to the start of school. 

  • NEW -  TdaP (Tetanus/Diphtheria/Acellular Pertussis) 

3.  State law requires all students to have either an up-to-date immunization document or a signed exemption on file.  Parents may sign a personal or religious exemption available in the school office, or they can contact their physician to obtain a medical exemption.  In the event of an outbreak, exempted persons may be subject to exclusion from school and to quarantine.
