Student Directories
Each year PTSA creates a
Student Directory. All PTSA members receive one (1) free directory and
may order extra copies for $2 each. If you are not a PTSA member, you may
also order a copy for $2.
How do I submit my
information?
Download the form below and
return it to the office along with any necessary payment no later than
September 10th. You may also submit your information online. If
submitting info online, any necessary payment is also due no later than
September 10th.
Paying for your Student
Directory:
PTSA Members - Your
first copy is free. Additional copies are $2 each. Send payment
with form or note on check memo line that you submitted info online. Pay
by one of these options.
1.)
Payment can be dropped off
in the Front Office in an envelope marked "Student Directory."
2.)
You can mail your payment to HMS Attn: Directory, 2455 Mesa Rd.,
Colorado Springs, CO 80904
Non-PTSA: All copies of the directory
are $2. (see above)
What if I just want my
Information in the Directory, and do not want to order a copy?
We will be happy to include
your information. Just submit your information and be sure to check the
appropriate box on the form that states you are "Submitting Information
Only."
