 Text Box: Detach this portion and return with payment to the front office:   My child ______________________________has my permission to participate in chess club directly after school in Room 30 (school conference room) on Tuesdays from 2/5/2008 through 04/29/2008.  My check payable to Chipeta for $15.00 is attached.  I understand that I will need to come to Room 30 at 3:30 to pick up my child after Chess Club.   ____________________________________           ____________________ Parent Signature                                                                    Date Text Box: Chess Club will begin February 5th and will continue every Tuesday through April 29th.  The fee will be $15.00 for the entire program (all three months).   Chess Club is open to all students (Kindergarten through 5th grade) and all skill levels.  No prior chess experience is required.  All materials provided.
